Our clinic hours are Tues-Fri 10am to 6pm and Saturday 10am to 4pm.  To request an appointment day/time, please fill out the form below.  Simply indicate your preferred day and time, and we will contact you to confirm your request.  (It may be necessary to reschedule if your requested day/time is not available.)
